Plastic Surgery for Men

John Joseph Conway is living proof that men can become just as vain as women when it comes to getting plastic surgery. The Chicago firefighter took his admiration of Bruce Willis to new heights when he traveled last month to New Delhi, India to get his jaw line reconstructed to look like the actor.

"I'm a firefighter. I need to look the part. I wanted to improve my jaw line. Bruce Willis has a nice strong jaw," he tells the Hindustan Times newspaper. Conway, who also traveled to India in 2005 for eyelid surgery, is so pleased with his results that he has already decided to bring his mother and sister to India for facelifts, where the cost will be $35,000 cheaper than the U.S. price tag.

While Conway has made international headlines for his plastic surgery trek, he is not alone in his masculine quest for body enhancements. According to the American Society for Aesthetic Plastic Surgery (ASPS), more than 900,000 men had plastic surgery procedures done in 2005. That year, the top five procedures for men included: lipoplasty (liposuction), rhinoplasty (nose reshaping), blepharoplasty (cosmetic eyelid surgery), male breast reduction and facelift.

Though men of all ages are now getting plastic surgery, the majority are men over 40. In a recent CBS News report, doctors indicate two main reasons men get plastic surgery. Males, they say, want a more youthful appearance to compete with younger colleagues in the workforce. And, they note, those who are divorced and dating again want to make themselves more appealing for the opposite sex. Whatever your reasons, here are a few things to consider about these procedures before scheduling your surgery.

Liposuction, one of the most popular procedures among men and women, is when excess fat is removed from below the skin by suction. However, removing so much fat can leave the skin very loose to hang from the body. This leaves the person with a very unsightly appearance and can be quite challenging for the patient to manage on a day-to-day basis. Additional surgery is often times required to cut off the excess skin. In some cases, men do not have to have this additional surgery because of their good skin elasticity, which enables their skin to adjust to the smaller dimensions with relative ease compared to women, whose skin is the opposite.

There's even surgery for men who desire a more defined look. With abdominal etching the doctor is able to create a muscular, rippled appearance for the abdominal region. Because this procedure is still relatively new, the ASPS warns men to seek out an experienced board-certified surgeon who has received adequate training. The same holds true for calf and pectoral implants, which are also growing popular among men. These surgeries require implementing a synthetic substance like silicone to fill out the area. According to the ASPS, calf implants were originally developed to restore leg contour in accident or polio victims. And pectoral implants were used to rebuild the chest of those with Poland syndrome, a congenital disease where the pectoral muscles are malformed or have collapsed into the chest.
For men more concerned with what's on their head, there are facial implants and hair replacement surgeries. Facial implants, which also use synthetic materials for enhancement, can be constructed around the chin and cheeks to create a more masculine cheekbone and jaw line. Hair replacement surgery involves removing small pieces of one's own hair and relocating it to a bald or thinning area. While there have been relatively positive effects with hair transplants, men have to have patience with the process. According to ASPS, a full transplant can take anywhere from 18 months to two years.

And for those who want "less intensive" treatments, there is chemical peeling and dermabrasion. Chemical peels use a combination of phenol, trichloroacetic acid (TCA) and alphahydroxy acids (AHAs) to diminish wrinkles and even skin tone, while dermabrasion involves surgical scraping to remove the old layer of skin. After the procedures, patients are usually left red during the healing stage. The ASPS advises men who feel self-conscious about this side effect to avoid alcohol for about four weeks because alcohol can make one appear even more flushed.

Despite the perceived vanity of some of these procedures, some men are getting plastic surgery as a necessity. Take twenty-something Chad Hummel for instance. Hummel severely damaged his right hand while operating a grain auger on his farm. In addition to extensive nerve damage, doctors had to amputate most of his fingers. The outlook seemed bleak. "If things weren't better, we would have to look into amputation and prosthetics," says Hummel in a recent interview with ASPS. Fortunately, an ASPS surgeon stepped in to help. Considering the extensive damage, the surgeon was able to repair Hummel's fractures and dislocations, perform nerve grafting, and add skin flaps to close the wounds.

Patients, however, should not expect surgery to fix it all. Whatever your reasons may be for getting surgery, you must still do your part to gain the best results. Contrary to popular belief, outcomes depend on both the doctor and patient. Andrew, a real estate attorney who had a nose job, botox and liposuction, agrees. "Keeping fit is a team effort," says Andrew in an ASPS interview. "He does his work, and I do mine. I work out everyday. I have a good eating-and-workout method that maximizes outcomes in a short time. You can't go under the knife and expect it to be a cure-all."

A Study Of Life After Gastric Bypass Surgery

With obesity rates continuing to rise in many Western countries people are becoming ever more sensitive to the health risks associated with being substantially overweight and of the increased risk of death resulting from obesity. But to what extent are these risks lessened by obesity surgery?

A recent study looked at the histories of a substantial number of individuals who underwent gastric bypass surgery in the 10 years from the start of 1995 to the end of 2004 and discovered that roughly one percent of patients died within twelve months of their surgery and that approximately 6 percent died inside 5 years. When these statistics were adjusted to account for age and sex and matched against figures for the population at large they were discovered to be fairly high. So what does this say about the ability of obesity surgery to reduce the general risk to our health?

In order to find the answer this particular question then we have to look beyond the headline figures and look at just why these deaths happened and where the true difference lies between weight loss patients and the general population.

If you examine the detailed figures two particular things stand out.

The first is the figure for people whose deaths resulted from heart disease which is the leading cause of death in the weight loss patients and is well above that seen in the population at large.

The second is the figure for deaths which resulted from suicide and from drug overdoses which, although not officially recorded as suicide, must nevertheless raise the question of whether these overdoses were in fact accidents. In the population at large you would expect to find approximately 2 deaths from suicide in a group of the same size as the study group and yet this study group showed a total of 30 deaths from suicide and drug overdoses.

If we look at these findings and consider them alongside our wider knowledge about individuals undergoing obesity surgery then we might perhaps explain this variation at least in part.

In spite of the fact that obesity surgery is frequently very successful it is often not undertaken until individuals are at risk from other conditions or comorbidities and, while surgery may cure several of these conditions and reduce the risks from others, many individuals are still at some degree of risk after surgery. In many cases for instance people continue to be troubled by such things as diabetes mellitus and high blood pressure and it is possibly unsurprising that this section of the population remains at higher risk from heart disease.

Further, while gastric bypass surgery may result in a significant loss in weight many people are still very much overweight for a long time after surgery and many individuals will remain so for months or years to come.

Lastly, the changes in lifestyle following surgery can be dramatic and many individuals experience depression in the months following their surgery. Without doubt much attention is paid to the physical affects of weight loss surgery and the requirement for such things as a strictly controlled diet and exercise but, all too frequently, very little attention is paid to the psychological affects of surgery.

Time will reveal the extent to which this explanation holds water but there should be little doubt that improved post-operative care for weight loss patients could go a long way towards finding a solution for this difference.

Minimally invasive or laparoscopic surgery as it also referred to, has become very popular in the U.S. and around the world in the past two decades. Breast Augmentation 101

According to a study performed by the American Society for Aesthetic Plastic Surgery, 364,610 breast augmentation procedures were performed in the United States in 2005 alone. This represents a nine percent increase from 2004's stellar numbers.

Used to increase, correct, or reconstruct the breast for cosmetic or post-operative purposes, there are many options available when considering breast augmentation. The first decision is the type of implant: Silicone or saline? Second is incision. To date, there are four common incision points to choose from. The final consideration is the placement of the implant: where is it going to be put within the breast? Because every person's needs and desires are different, the plastic surgeon will generally advise the patient on the optimal augmentation procedure.

There has been some controversy over the choice between saline and silicone implants. Saline implants are considered the safest and most popular choice to date, representing 83.4 percent of implants in 2005 and carrying FDA approval since May of 2000. On the other hand, silicone implants, approved by the FDA in November of 2006 after a 14 year ban arising from links between silicone implants and auto-immune illnesses, are considered more natural looking. Whereas sub-glandular insertion places the implant beneath the breast tissue, but above the muscles, a sub-muscular placement sets the implant partially beneath the pectoral muscles. There are positives and negatives to both options. Situating the implant sub-muscularly costs more, requires more anesthetic and healing time, but it is also the most suitable option for women who have very small breasts. Furthermore, because a sub-muscular placement interferes less with mammograms, it is also the best choice for individuals with a history of breast cancer.

Though it's an outpatient procedure which takes only 1 to 2 hours to complete, breast augmentation surgery does require significant downtime. Side effects such as swelling, discoloration, fatigue and discomfort should be expected. Most patients will not be able to shower or work for several days and exercise should be avoided for about a month. As a rule, it is important for individuals with breast implants to have regular checkups and mammograms.

For women who are simply looking for perkier, firmer breasts with or without an implant, the breastlift or mastopexy is a good option. This surgical procedure raises and reshapes sagging breasts by removing excess skin, lifting the breast and relocating the nipple. Not unlike breast implant procedures, a mastoplexy does leave scars which are usually obscured by a bathing suit. Furthermore, having a breast lift won't keep you firm forever; pregnancy, aging, and fluctuations in weight are likely to effect just how long the results will last.

Plastic Surgery: Where Science Meets Art

Today, plastic surgery is synonymous with Hollywood and its never ending quest to remain young and beautiful forever, through mammoplasty (breast reduction or augmentation), mastopexy (breast lift), rhinoplasty (nose lift), chin augmentations, cheek augmentations, tummy tucks, liposuction, and face lifts. These procedures are more commonly referred to as cosmetic surgery. However, the art and science of plastic surgery is not solely limited to improve the physical appearance of an aging Hollywood actor. In fact, this particular branch of surgery was developed to correct disfigurement and restore impaired function. This type of surgery became known as reconstructive surgery, which can be defined as the correction of a congenital or acquired deformity; as opposed to cosmetic surgery, which entails the correction of a perceived physical imperfection.

The use of the word plastic does does not refer to the synthetic polymer material, but is derived from the Greek word "plastikos," meaning to mold or shape. This refers to the fact that plastic surgeons typically mold and reshape bone, cartilage, muscle, fat and skin to achieve the aesthetic results they desire. These body parts may be moved to fill, alter, or cover the appearance of a defect, or completely removed.

Although rudimentary plastic surgery methods were being practiced as early as 800BC, the techniques being used today are largely attributed to Sir Harold Delf Gillies, a Cambridge-educated surgeon from New Zealand. He is often referred to as the father of plastic surgery. Gillies joined the Red Cross during World War I and saw the need for a plastic surgery unit for the British troops after seeing the French surgery units in Paris. This war was particularly gruesome because it had the distinction of being the first fully mechanized war, and the advent of heavy artillery, shells and machine guns meant more devastating injuries than ever before could be inflicted. The War Office recognized the necessity for a specialized unit of surgery and subsequently sent Gillies to the Cambridge Military Hospital in Aldershot to set this up, thus becoming Britain's first plastic surgeon. Soon men began pouring in to the facility from the field hospitals, all with jaw and face injuries that required reconstruction.

Gillies was determined not only to restore the function of these wounded men but to also make the person look as normal as possible, if not more attractive than before. He called his brand of plastic surgery, " a strange new art." This melding of function and aesthetics underlined his entire body of work. Gillies pioneered many surgical techniques and developed instruments to use in his operations; many of them are still being used today. One of these techniques is the pedicle tube, in which a skin graft taken from an undamaged part of the patient's body is sewn into a tube and used to cover the damaged area. Not only was the blood supply maintained to the skin graft, but sewing the edges of skin together meant that the skin graft was less prone to infection, a constant hazard to patients.

He delighted in sharing his skills and insights with doctors from all over the world, helping many countries to establish their skill base for this emerging field of surgery. In addition to describing his work in written form, he was the first surgeon to make pictorial records of facial reconstruction cases, both before and after surgery. In 1920 he published a book called Plastic Surgery of the Face, which detailed his work and techniques.

As weapons of war become more and more destructive, the need for reconstructive surgery increases for its casualties. But because of men like Sir Harold Gillies, these men and women can get back the face that was stolen from them.

Out of the 30 surgeons I have investigated, I have only kept 3 that I bring groups to on a regular basis. I have worked with them since 1997 and I have never been able to catch them cheating.

One of them took my hands one day, drew some symbols in them and said a prayer. Then he moved my fingers above the body of a patient who was lying on the couch. As my fingers moved a few inches over the body it opened and blood appeared. The body then closed as my fingers moved on. This happened with my own fingers, not those of the surgeon. How could he cheat this?

I have also been allowed to pull tumours that were in the patient whilst his body was kept open for me. Not the hands of the surgeon but mine.

In the Philippines, there are only few training institutions with plastic surgery program that are accredited and recognized by the local association of surgeons (Philippine College of Surgeons, Philippine Board of Plastic and Philippine Association of Plastic Reconstructive and Aesthetic Surgeons). Nowadays, there are still a lot of doctors (even non doctors), though board certified, but definitely not in plastic surgery, who perform plastic procedures. With the complexity of the plastic surgery specialty, it is not enough for a physician to just undergo months or even a year of observership or training in an unaccredited establishment, in order for them to comprehend all the principles and applications in the field of plastic cosmetic surgery.

3. A surgeon, equipped with a firm foundation in Cosmetic/Aesthetic Plastic Surgery should be able to evaluate whether you are a good candidate for the surgery you are considering. He must be able to discuss with you, all possible options and its associated risks and advice you which procedure is best to attain your desired results. If there are less invasive or non-surgical procedures that could fulfill your needs, your surgeon should be knowledgeable about this and be able to tell you its advantages and disadvantages. Please be aware that there is no such thing as standard cosmetic procedure that would fulfill the desired result of all patients. Each patient should be approach and treated individually, since we have our own personal definition of beauty.

4. Do not look for the cheapest doctor, more so ,do not be deceived by unrealistic offer. Know your surgeon’s training and academic profile. Take note of his offer, if it is safe and attainable. You could inquire from any physicians’ association, regarding the authenticity of your surgeon’s specialty or training; lastly, ask friends and previous patients regarding his credentials. Try visiting their website, if he has one. Your surgeon must be able to provide you details of his training, specialization and the number of years he underwent training.

5. Your surgeon must be able to extensively discuss with you all your possible options. Don’t hesitate to ask questions, a good plastic surgeon must be able to entertain all your inquiries regarding your desired procedure. Through this, the surgeon can establish rapport with their patients and establish a comfortable environment.

6. Don’t allow yourself to undergo liquid silicon injection at your nose, breasts, or any part of your body. The use of silicon oil injection has long been abandoned and condemned because of its unpredictable result and complications, not to mention its untoward results such as extrusion and infection. Your doctor must be able to provide you data or clinical study regarding the use and safety of the product he is trying to promote.

7. Don’t be attracted to newer procedures, drugs and prosthetics without proven long term studies with regards to result, efficacy and safety. Inquire about its approval for use by local and foreign regulatory agency such as US-FDA, Department of Health, etc. Take into consideration the quality, safety and efficacy rather than the cost.

8. Take note if your surgeon’s clinic or cosmetic center were able to meet certain standard of safety and certified by the local Health Department. But be informed, that there are certain procedures that are best done in a hospital setting.

9. Ask your surgeon regarding affiliations or membership to any local Plastic Surgery organization that is recognized by both local and international body of Plastic Surgeons. Breast Augmentation/ Breast Enlargement

The field of cosmetic plastic surgery is a dynamic branch of medicine which continues to grow through modern innovation, technique and researches; this is because of the endless desire for physical perfection by man. Breast Augmentation for example, is now one of most frequently performed cosmetic plastic procedure in the world. The interest for breast enhancement by patients and surgeons alike, continue to evolve as newer methods with improved result are being develop. At present, more and more women undergo breast augmentation every year as the popularity of the procedure is increasing, especially among women in the entertainment industry.

Silicon shelled implants are commonly used for breast enhancement. This implant could be either silicon gel or saline-filled. The implants are used to augment the breast of an individual that lack breast tissue or volume that could be genetically predisposed or environmentally induced. The female breast tissue is primarily composed of gland and fat cells and in the past, surgeons tried to inject the breast with fat that was initially obtained through liposuction from other parts of the body. A breast implant is a sac of silicone elastomer that is either filled with saline or silicone, and is surgically placed under the breast tissue or chest muscle. Silicon oil injection, which used to be performed in the past for breast augmentation had been condemned locally and internationally, since the result are unpredictable and compounded by numerous complication such as extrusion, skin necrosis and infection. Do not allow youreself to be subjected in this kind of procedure, because of its risks and deforming results.

The selection of breast implant size or volume depends on multiple factors such as the proportion of the breast to body height and shoulder width, as well as the location of nipple/areola complex, and cleavage. And because the concept of breast aesthetics are constantly changing, there are many factors such as implant type, size, and incision site that should be considered to tailor the needs of each individual specifically. The incision for implant placement may be placed at various locations of the chest/breast area such as; under the breast or at the breast fold; around the nipple areolar area; in the armpit (axillary); and the latest, at the umbilical area via endoscopic approach. The resulting scars are thin and inconspicuous, especially at the armpit area. Each of this incision has its advantage and disadvantages that must be discussed thoroughly between the patient and surgeon.

Breast implant could be silicone gel or saline filled. Silicon filled implant was popularly used 12 years ago in the United States, until it was thought that it possibly causes autoimmune mediated health problems associated with leakage, that’s why saline implants is now used in most of breast surgeries. At present, silicone implants are now gaining momentum and recent studies had come out, proving the safe use of silicon breast implant in breast enhancement surgery. The advantage of silicone implant is that it is softer and appears more natural. However, leaks may be found in surrounding tissues. Generally, saline is a bit firmer, but it is without the supposed health risks because a leak is simply absorbed by the body.

The final breast size is mainly a personal decision by the patient. There are several ways to estimate the ultimate breast size. This would include the breast cup size, but this is an inexact estimate of the final size, since cup sizes vary with the bra manufacturer and how tight or loose the patient wears her bra, as well as how much breast tissue is already present. But describing the preferred cup size does aid the surgeon. The most accurate method of estimating the implant size is to fill a plastic bag with measured amounts of water or breast implant sizer that is place in a bra with the patient's estimated cup size. The eyes are the window to the soul and in Chinese face reading, the shape and form of the eyes may indicate your fortune and can predict your relationship with your children in the future.

It is believed that if one changes the feature to something much less than what they started off with, their abilities in their work life and their efforts to make money will be significantly sabotaged.
